COMPOSITION AND DOSAGE FORM

Injection solution:

Pharmacological action

Short-acting opioid analgesic. By analgesic effect 100 times greater than morphine. The maximum effect of the drug in in / in the introduction develops in 1-3 min. The duration of action with a single dose – 20-30 min. When given at high doses causes a slight transient inhibition of the respiratory center.

INDICATIONS

* Premedication prior to surgery;
* Implementation of the induction of anesthesia (in conjunction with droperidolom and / or nitrous oxide);
* Postoperative anesthesia;
* Neyroleptanalgeziya (in combination with droperidolom).

Dosage regimen

Adults for sedation, as well as in the postoperative period Fentanyl injected i / m at 50-100 mg (1-2 ml). To carry out induction of anesthesia drug is used in / at a dose of 100-200 mg (2-4 ml).
For neyroleptanalgezii Fentanyl given intravenously in a dose of 200-600 mg (in combination with droperidolom), fractional, every 20 minutes, under the control of respiration. At higher doses (50-100 mg / kg body weight) Fentanyl is used during open heart surgery.

ADVERSE EFFECTS

Since the cardiovascular system: often – bradycardia; possible – hypotension.
After the introduction of high doses or in case of rapid i / v injection of the drug, there may be respiratory depression, muscle stiffness and rigidity of the chest, in some cases possible bronchospasm.

CONTRAINDICATIONS

* Caesarean section (before the extraction of the fruit);
* Increased intracranial pressure;
* State, accompanied by inhibition of the respiratory center.

Pregnancy & Lactation

During pregnancy, the drug should be used with extreme caution, if absolutely indicated. If necessary, the appointment during lactation should stop breastfeeding for a period of its application.

Cautions

The drug should be used only by highly qualified personnel in specialized hospitals.
In the postoperative period for patients should be set closely monitored. In patients with reduced body weight, during long operations, in cases of frequent repeated application may increase the duration of fentanyl.
Precautions should be applied while taking the drug GCS, antihypertensive drugs and insulin.

OVERDOSAGE

Symptoms: bradypnea, apnea, respiratory depression, muscular rigidity.
Treatment: a specific antagonist is naloxone. When respiratory depression and muscle rigidity was also used muscle relaxants and mechanical ventilation. To eliminate bradycardia using atropine. In the case of arterial hypotension prescribe parenteral administration of fluids.

Drug Interactions

With simultaneous use of nitrous oxide enhances muscle rigidity, callback fentanyl. With simultaneous use of tricyclic antidepressants, as well as funds, which have a depressing effect on the central nervous system, increase the side effects of fentanyl on the central nervous system (in particular, respiratory depression).
